ORDERING INFORMATION
Numbers on online order form and order processing envelopes correspond with numbers in instruction guide.
2 If you are submitting more than one diskette for duplication please use one envelope for each diskette. Label each envelope in Section 2 with the diskette number of the one inside and the total number being submitted for duplication for this order. For example: you have three diskettes and you 're using three separate envelopes. . . envelope one would be labeled 1 of 3, envelope two would be 2 of 3 and so on.
3 If you are submitting more than one diskette for duplication and want the diskettes collated in order and placed into a poly bag. . .choose Yes in Section 3. If you selected Yes, indicate the order the diskettes are to be collated by placing the correct sequence in the boxes in Section 3. The box on the far left will be the top of the collation order.
4 Indicate how many copies you wish made for each diskette master contained in each envelope. There is a minimum requirement of 50 diskettes per master. A $30.00 per master below minimum charge will be app1ied for any request of less than 5O copies. Select your diskette color by checking the correct box.
Pleose note that all diskette copies will be the same size and track density as the master being submitted
5 If you wish to have labels made up and applied to your diskettes select the Yes box in Section 5. You have a choice of using dot matrix labels which are supplied at no charge, or may select laser labels which cost 4 cents each. Laser labels may include a corporate logo or company mark along with one of four available fonts, to provide a semi-custom label at very little cost. If you wish a corporate mark or logo placed on your label, enclose a business card or letterhead containing your logo and we'll scan it in.
Laser labels are black and white only. The clarity of your logo will depend on how clear and complex your submitted art material is. Some logo's can't be scanned.
6 Please indicate how you wish your copied diskettes returned by checking the appropriate box.
See the Shipping ond Handling section to determine the per diskette shipping cost.
7 Please read and initial the Retention of Master Section of Section 7. This states that you are not submitting your only master for duplication. On-Line Copy Corporation (and their authorized retailers) shall not be responsible for any loss or damage to your master for any cause. In addition, you must sign the Customer's Warranties and Indemnification statement which essentially says you have the right to copy what you are submitting and that you shall indemnify and hold On-Line Copy Corporation (and their authorized retailers) harmless from any and all claims arising from the copying of copy protected and/or patented software.

Computing Costs.
If you ore submitting multiple envelopes, use the last
envelope to compute your total job cost.
Determine how many total copies from all masters you are going to require. That number goes into Box A. For example: envelope one calls for 300 copies of 3.5"DSHD and envelope two calls for 200 copies of 3.5"DSDD. The total number of copies indicated in Box A should be 500. Box A is used for computing laser label, collation and delivery costs.
Each diskette type has a different copy price. In the example above, if envelope one contained a 3.5"DSHD diskette master, look at the 250 to 499 price under 3.5"DSHD diskettes to get the unit price when duplicating 300 units. Place that number in the box adjacent the 3.5"DSHD and then multiply it by 300 to obtain the total cost for that type of diskette. Note: if you had another envelope with 3.5"DSHD diskettes, add them together to obtain a total type diskette cost.
Bagging (poly bagging) costs are obtained by determining how many collated and bagged sets you are ordering and then multiplying by 18 cents per set. For example:you are ordering a 4 diskette set, your bagging charge is $18.OO for 100 sets of 4 diskettes each. The total job, however, calls for 400 diskettes.
Delivery Costs are computed by determining which method of return shipment you require. Obtain the shipping cost for each diskette from the Shipping and Handlingchart and multiply that number by the number in Box A.
Add all of the boxes together that contain the $ sign and place the total in the Total Order Cost box. Add sales tax if applicable. This is your total order cost.
9 Use this section to tell us what you want printed on your label. Print carefully using upper and lower case letters. Write right on the envelope (or type directly into the online form). The label that is the taller of the two is the 3.5" diskette label and the other one is for 5.25" diskettes. Circle letters or words you want to be bold. Select your choice of font by checking the appropriate box. (Include logo sample if required).
Note: the label information should correspond to the diskette contained in its respective envelope only!

In paper form, this form exists on an envelope that is used to submit disks. The customer could print out this web form and use it to cover the envelope, or use it as a guide to the envelope form itself. Or the customer could fill this out as an online form which could supplement the disk submission.
